FERPA Forms

Students have the choice to share some or all of their FERPA-protected information with parents and guests. FERPA authorization to release information forms are required for information to be shared.  

Please note that there are two types of forms. The Dean of Students FERPA is for information relating to disciplinary records whereas the Registrar's FERPA is for information relating to academics.

FERPA Form - Dean of Students Consent to Release Information (ONLY for disciplinary records)

 The Dean of Students FERPA is for information relating to disciplinary records. Our department is unable to provide information relating to academic standing. Please contact the Registrar's Office for this information.

Notice of Advisor/Attorney Form

This form is required to be submitted if you are meeting with our staff and you have an advisor/attorney present. Any student or student organization who is assisted by an advisor during Student Code of Conduct proceedings must submit this completed form to the Dean of Students Office at least five (5) business days prior to the scheduled hearing.

FERPA Form - (ONLY for Academic Records) - Registrar's Authorization for Release of Information (E-form on UAccess)

This form is intended for use at the Office of the Registrar to get information a student's academic status. Filling out this FERPA form does not disclose disciplinary records.
