Record of Appeal to College Dean (College Dean only)

If you received the appeal, please follow the instructions under the Academic Integrity Procedures III, once the form if completed, send it to the faculty member and DOS-Codes@email.arizona.edu. 

Sanctions for Multiple Violations (Academic Dean only)

If the student has multiple violations, please follow the instructions under the Academic Integrity Procedures II, once the form is completed, send a copy to the faculty member's dean and DOS-Codes@email.arizona.edu. 

Request for Appeal to Dean of the College

If you wish to appeal an academic integrity violation, please complete the form above and send it to DOS-Codes@email.arizona.edu.

Request for Appeal to the University Hearing Board

You can only appeal to the University Hearing Board if the sanction imposed is suspension, expulsion, permanent notation on transcript, or revocation of degree AND you have been found in violation of the Code of Academic Integrity.
